For patients with moderate to moderately severe chronic pain who do not need instant pain relief, the best way to take ULTRAM is to gradually increase the dose until the least effective dose is determined. The dosage should start at 25 mg per day and be increased in increments of 25 mg every three days until the patient is taking 100 mg per day. The medication should be taken in 25 mg doses throughout the day, so that a patient taking 100 mg takes 25 mg every 4 to 6 hours. After that, the dose can be increased by 50 mg per day taken in separate doses until a dose of 200 mg per day is reached. The maximum dose is 400 mg per day taken in separate 100 mg doses every 4 to 6 hours.
Patients who require rapid pain relief can start out with 50 mg to 100 mg doses if it is determined that the benefits of quick pain relief outweigh the potential risks starting out with a higher dose.

Individualization of Dose
As with any effective and safe regimen for managing pain, dosage should be tailored to fit an individual patient’s need, and the lowest effective dose should always be used. Adult patients who start at lower doses that are gradually increased are less likely to stop taking the medication and more likely to benefit from the medication without experiencing adverse side effects.

Patients with a creatinine clearance less than 30 mL/minute should not take ULTRAM more frequently than every 12 hours and should not take more than 200 mg per day. Patients on dialysis can take the usual amount on the day of dialysis as only 7% of the medication is removed in the process.

Patients with cirrhosis should be limited to doses of 50 mg every 12 hours.

Patients over 65 years of age should use caution and take the lowest effective dose because of the increased incidence of liver, kidney or heart conditions and the likelihood of taking other medications at the same time. Patients over 75 years of age should not take more than 300 mg of ULTRAM per day.
